About the Role

We are hiring two Bilingual Field Case Managers to support patients, healthcare professionals, and clinics across Atlantic Canada and Ottawa/Southwestern Ontario through a growing Patient Support Program (PSP). As a Field Case Manager, you will serve as a key point of contact for patients, physicians, clinic staff, and other healthcare stakeholders throughout the treatment journey. You will support patient onboarding, facilitate access to specialty therapies, provide education and adherence support, and help ensure an exceptional experience for healthcare providers and patients.

This is a field-based role requiring regular travel within your assigned territory and offers the opportunity to play an important role in helping patients access and remain on therapy while building strong partnerships within the healthcare community.

What You’ll Do
  • Serve as the primary program contact for physicians, clinic staff, and patients enrolled in the Patient Support Program.
  • Support patient onboarding and program enrollment activities.
  • Facilitate timely access to prescribed specialty medications and therapies.
  • Provide disease-state, treatment, and medication education to patients.
  • Support treatment adherence and ongoing patient engagement.
  • Conduct patient chart reviews and collect information required for reimbursement and patient access activities.
  • Collaborate with physicians, clinics, pharmacies, and internal stakeholders to coordinate care and resolve barriers to treatment.
  • Maintain accurate and timely documentation within program systems.
  • Escalate patient, reimbursement, or operational concerns as appropriate.
  • Build and maintain strong relationships with healthcare providers and clinic teams throughout the territory.
  • Provide updates to physicians and clinic staff regarding patient progress and program activities.
  • Participate in program initiatives and continuous improvement activities.
Basic Requirements
  • Fluency in French and English (written and verbal).
  • Previous experience working within a Patient Support Program (PSP).
  • Postsecondary education or an equivalent combination of education and experience consistent with the P1 Professional Career Framework.
  • Strong communication, organization, and relationship-management skills.
  • Proficiency with Microsoft Office and digital documentation systems.
  • Ability to work independently while managing multiple priorities.
  • Valid driver’s license and ability to travel regularly throughout the assigned territory.
  • Ability to work within program operating hours of 8:00 a.m. to 8:00 p.m. EST.
  • Ability to maintain a secure home office environment suitable for confidential patient conversations.
Preferred Skills & Experience
  • Nursing designation (LPN, RPN, RN, or equivalent).
  • Experience supporting specialty pharmaceutical, biologic, or patient access programs.
  • Experience in immunology, rheumatology, dermatology, gastroenterology, or other specialty therapeutic areas.
  • Knowledge of reimbursement navigation, patient access, and specialty medication support.
