About the Role
Our client seeks a proactive Community Health Navigator to join their Community & Social Care team in Port Elizabeth . This role is crucial in bridging the gap between community members and essential health and social services. You will act as a vital link, helping individuals and families understand and access the care they need, overcoming barriers such as language, culture, or socioeconomic status.
This position offers a rewarding opportunity to empower individuals and improve health outcomes within the Eastern Cape community, working collaboratively in a hybrid model that balances field engagement with remote coordination.
- Conduct needs assessments to identify health and social service requirements of community members.
- Educate individuals and families about available health resources, preventive care, and wellness programs.
- Assist clients in navigating complex healthcare systems, including appointment scheduling and insurance queries.
- Facilitate referrals to specialized medical care, mental health services, and social support agencies.
- Advocate for clients' needs within healthcare settings and social service agencies.
- Develop and maintain strong relationships with local healthcare providers and community organizations.
- Bachelor's degree in Public Health, Health Sciences, Social Work, or a related discipline.
- Proven experience in community health, patient navigation, or social services.
- Strong understanding of the healthcare landscape and social support systems in Port Elizabeth.
- Excellent interpersonal, communication, and cross-cultural sensitivity skills.
- Ability to work independently and manage a caseload effectively.
- Valid driver's license and access to reliable transportation for community visits.
