The role:This is an excellent opportunity for a Band 6 Physiotherapist. We are looking for a motivated and experienced individual to join our community rehabilitation service, a dynamic multi-disciplinary team providing assessment, treatment, and intervention to older people and disabled adults with multiple pathologies within South Locality of Glasgow City.
The Rehab service responds to a wide range of referrals from acute and community settings with the aim of preventing hospital admission and maximising patients independance within their own home. You will be part of an integrated team, working directly alongside other AHP and nursing professions to deliver person centred goals.
As a physiotherapist, you will deliver a high quality specialist service, providing expert clinical intervention, education and support to patients and their carers. You will provide specialist patient assessment, case coordination, treatment and plan discharge for adults with complex rehabilitation needs.
All other roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern:- Castlebank Social Work Dept
- Fixed-term Contract for 12 months
- Part-time position (26 hours per week)
- Shift pattern for this post are Monday to Sunday, 08:30 - 16:15
Please note the salary is Pro Rata for part time hours.
Key responsibilities:The post holder will work autonomously in a diversity of health and social care settings taking on, in addition to the professional clinical role, case co-ordination, health promotion, audit projects, staff /student supervision / induction and the provision of expert advice on Professional interventions. This includes;
- The provision of specialist direct clinical care, encompassing professional assessment, treatment and management of patients referred to the Service.
- Working together with other professionals in health, education and social care to provide the most effective care to adults with a diverse range of clinical conditions.
- To provide supervision of less experienced related professionals as well as support workers and students. Advise members within the interdisciplinary team and other agencies regarding their professional intervention.
- To work autonomously to manage a delegated caseload of patients to assess, develop, implement and evaluate interventions
- To assist in the support of patients discharged from hospital and prevention of inappropriate readmissions to hospital.
- Assist in the assessment and maintenance of patients within the community and prevent inappropriate admissions to hospital.
- Work according to the needs of the Service which will include varying shift patterns and weekend working rotas.
- Contribute to and support Intermediate Care and the Reablement process to a defined patient group.
Essential Criteria
- Degree/Diploma or equivalent qualification in Physiotherapy.
- Registered with the relevant Health Professions Council (HCPC).
- Excellent communication skills.
- Current full driving licence.

- Post-graduate experience, including relevant experience in one or more of the following areas:
- Adult Rehabilitation
- Neurology
- Orthopaedics
- Respiratory Medicine
- Care of the Elderly Specialities
- Ability to work independently.
- Experience of working within a multidisciplinary community-based team.
- Leadership skills.
- Knowledge of IT systems.
- Evidence of Continuous Professional Development (CPD).
