• Conduct virtual needs assessments to identify barriers to healthcare access and develop tailored action plans.
• Maintain accurate and confidential client records and document all interactions and referrals.

• Bachelor's degree in Public Health, Health Sciences, Social Work, or a related field.
• Minimum of 2 years of experience in health navigation, community health work, or a similar role.
