- Conduct comprehensive needs assessments for elderly clients, evaluating physical, emotional, and social well‑being.
- Develop, implement, and monitor personalized care plans, ensuring client goals are met.

- Bachelor’s degree in Social Work, Gerontology, Psychology, or a related field.
- Minimum of 3 years of experience in case management, preferably within elder care or geriatrics.
- Strong understanding of the challenges and needs of the elderly population and available support services in the Free State region.
- Excellent assessment, planning, intervention, and evaluation skills.
- Proficiency with case management software, telehealth platforms, and standard office productivity tools.
- Exceptional communication, interpersonal, and problem‑solving abilities.
