Your focus will be on addressing physical, emotional, and social needs through tailored treatment plans and strong therapeutic relationships.

Key Responsibilities:

• Conduct comprehensive client assessments

• Formulate client-centered goals and recommendations

• Plan and implement individualized treatment programs

Requirements:

• Graduate from an accredited Social Work program

• Member in good standing of Ontario College of Social Workers

• Recent experience in acute rehab or community care

• Strong interpersonal and organizational skills

• Ability to work independently within interdisciplinary teams
