Home Health Physical Therapist; PT

Pay: $75.00 - $90.00 per hour

Location: Concord, CA - Hybrid. This is a field-based home health position serving patients in their homes throughout the surrounding territory, with documentation typically completed in the field.

Note: Candidates must have an active California Physical Therapist (PT) license. Home health experience, HCHB/Homecare Homebase, OASIS experience, BLS/CPR certification, and prior field-based patient care experience are preferred but not required.

• Provide physical therapy services to patients in their homes.
• Evaluate patient mobility, strength, balance, and functional needs.
• Develop individualized treatment plans focused on improving independence and quality of life.
• Deliver therapeutic exercises and interventions designed to improve mobility and physical function.
• Educate patients and caregivers on exercises, safety, and strategies to support continued progress.
• Monitor patient outcomes and adjust treatment plans as appropriate.
• Collaborate with other healthcare professionals involved in the patient's care.
• Complete accurate and timely clinical documentation.
• Travel throughout the assigned territory to provide patient care.

• Active California Physical Therapist (PT) license required.
• Home health Physical Therapist experience preferred.
• HCHB/Homecare Homebase experience preferred.
• OASIS experience preferred.
• BLS or CPR certification preferred.
• Experience providing field-based or in-home patient care is a plus.
