Position Overview:
We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. In this hybrid role, you'll focus on providing Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services to patients managing chronic conditions such as diabetes, cardiovascular disease, and COPD. You'll play a vital role in supporting patients by offering personalized care coordination, ensuring they receive the resources, education, and support needed to thrive.

Key Responsibilities
- Serve as the primary point of contact for assigned patients, ensuring their needs are met with empathy and efficiency.
- Deliver CCM and RPM services in alignment with program guidelines.
- Accurately document all patient interactions and care plans in the EMR system.
- Meet or exceed metrics for outbound patient contacts, compliance standards, and case management.
- Perform data entry and ensure all interactions are documented accurately.
- Ensure compliance with regulatory guidelines and policies.
- Assist with delivering and setting up RPM devices as needed.
- Manage pre-authorizations, referrals, prescriptions, and medication reconciliation.
- Coordinate with external vendors and healthcare providers for comprehensive care.
- Provide education on medications, chronic disease management, and lifestyle modifications.
- Address patient inquiries, triage requests, and coordinate follow-ups.
- Support healthcare providers with medication adjustments and clinical concerns.
Qualifications
- Certification as CNA, CMA, RMA, Med Tech, or equivalent.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively.
- Proficiency in EMR systems and healthcare software.
- HIPAA-compliant remote workspace with reliable computer and internet.

Salary: $16.00 - $21.00 Per Hour
