Phamily, the core technology platform of Jaan Health, seeks a Patient Support Specialist to guide patients through enrollment in Phamily-enabled care programs. You will conduct high-volume outreach, educate patients on proactive chronic care, and help strengthen communication between patients, providers, and care teams.
This part-time role starts fully remote and will eventually transition to onsite work at our new Indianapolis office, reporting to the Enrollment & Consent Manager.
