Center Well Home Health is seeking a Home Health Physical Therapist Assistant to join our care team in Solon, Ohio. You will administer the physical therapy treatment plan to patients in their homes, under the direction of a Physical Therapist, focusing on functional improvement and independence.
You will document client records, participate in in-services, and ensure infection control. A valid PT license, driver's license, CPR, and ability to work with geriatric patients are required.
