Remote Care Manager: Person-Centered Care & Coordination

Bond Health in Albany is seeking Care Managers for a primarily remote, full-time role. You will deliver six core services, perform comprehensive assessments, and develop Life Plans with individuals and families.

In-person visits occur as needed, with regular coordination of benefits and services. Applicants should have a Bachelor's or Master’s degree with relevant experience, knowledge of HIPAA, and strong communication skills.
