Hybrid FNP​/PA: Remote Primary Care Lead

Hartford Health Care Medical Group is seeking a full-time Family Nurse Practitioner (APRN) or Physician Assistant (PA) to join our hybrid remote primary care team. You will deliver transitions of care, annual wellness visits, and same-day primary care visits remotely, with on-site support as needed.

The role supports care delivery across two Campus Care sites and remote patient management, with opportunities for collaboration, supervision, and professional growth within a large, integrated system.
