Hartford Health Care Medical Group seeks a Family Nurse Practitioner or Physician Assistant for a hybrid remote primary care role. You will perform transition of care, annual wellness visits, or same-day primary care visits, with on-site support as needed.
The position is full-time with a mix of remote work and in-person days at Campus Care sites, based on residence. CT licensure and APRN/PA training are required, with preference for 5+ years in primary care.
