Community Health and Wellness Navigator

Our client is looking for an experienced and proactive Community Health and Wellness Navigator to join their team in Nelspruit. This role is essential for guiding individuals and families through the healthcare system, connecting them with appropriate health services, and promoting preventative care and wellness programs. You will act as a key liaison between community members and health providers, ensuring accessible and culturally sensitive care.

This position offers a vital role in improving public health outcomes in the Mpumalanga region.

• Assess the health and wellness needs of community members.
• Provide information and guidance on accessing healthcare services, including medical aid and public health programs.
• Connect individuals to specialists, primary care physicians, and mental health services.
• Facilitate participation in community health education and wellness initiatives.
• Advocate for clients' health needs within the healthcare system.
• Maintain confidential client records and track service utilization and outcomes.

• Bachelor's degree in Public Health, Nursing, Social Work, or a related field.
• Minimum of 3 years of experience in community health, patient navigation, or healthcare advocacy.
• Strong understanding of the South African healthcare landscape and available resources in Nelspruit.
• Excellent communication, interpersonal, and problem-solving skills.
