We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Clinton Township and surrounding area.
You'll Do:
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Provide hand‑on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
- Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
- Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
- Work independently, but never alone
- Travel to patient homes within assigned territory
- Up to $20,000 in student loan repayment for CNA's advancing to LPN certificate after 1 year of employment

- High School Diploma or certificate
- Minimum one year healthcare experience
- Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
