About the Role:Amazing Care Home Health Services is seeking an experienced Adult Therapy Supervisor to provide clinical leadership and operational oversight for our adult home health therapy services in Colorado.
This role serves as a key connection point between our patients, field clinicians, care management, and operational teams. The Adult Therapy Supervisor is responsible for supporting a team of Physical Therapists, Occupational Therapists, and Speech-Language Pathologists while helping ensure patients receive timely, coordinated, high-quality care throughout the entire episode of service.
This is an ideal opportunity for an experienced home health therapist who enjoys developing clinicians, solving operational challenges, improving care delivery, and making an impact beyond an individual patient caseload.
What You'll Do:Clinical Leadership & Team Support
- Provide day-to-day clinical leadership, guidance, and support to a multidisciplinary team of PTs, OTs, and SLPs.
- Serve as a clinical resource for therapists navigating patient needs, changes in condition, care planning, documentation, and other clinical questions.
- Support clinician development through coaching, education, feedback, and ongoing communication.
- Promote clinical excellence, accountability, collaboration, and a strong patient-first culture across the therapy team.
- Partner with clinicians to identify and resolve barriers impacting patient care or successful service delivery.
- Support onboarding, orientation, and ongoing development of new therapy team members.
- Assist with patient visits and clinical coverage when needed to support continuity of care.
Patient Care & Case Management
- Oversee and coordinate therapy services throughout the patient lifecycle, from admission through discharge.
- Help ensure appropriate and timely delivery of therapy services based on patient needs and established plans of care.
- Collaborate with clinicians and internal teams to address changes in patient status, scheduling challenges, missed visits, or other barriers to care.
- Support effective case management and communication across disciplines.
- Collaborate with physicians, physician offices, care managers, field clinicians, patients, families, and internal partners to facilitate coordinated care.
- Support appropriate utilization of therapy services while maintaining a strong focus on patient outcomes and quality.
Quality, Documentation & Compliance
- Provide oversight and support related to clinical documentation, including OASIS requirements as applicable.
- Help ensure therapy documentation is timely, accurate, complete, and aligned with regulatory, payer, and organizational requirements.
