We are looking for a Clinical Pharmacist who resides in Missouri and holds an active, unrestricted Missouri Pharmacist license to join a Clinical Engagement Services Center supporting Medication Therapy Management (MTM), medication adherence, diabetes, polypharmacy, STARS adherence, retrospective drug utilization, and other clinical programs.
Location:Remote - Must reside in Missouri
Schedule:Monday-Friday | Rotating shifts between 9:00 AM-8:00 PM CST
Extended hours and occasional weekends may be required.
Key Responsibilities- Conduct telephonic consultations and comprehensive medication reviews with members.
- Review medications and clinical information to identify gaps in care, adherence concerns, drug interactions, high-risk medications, and therapeutic opportunities.
- Assess medication regimens for effectiveness, appropriate dosing, and potential adverse reactions.
- Provide patient education on medications and disease-state management.
- Communicate clinical recommendations to members and healthcare providers.
- Accurately document patient care, interventions, and recommendations.
- Maintain HIPAA compliance and meet established productivity and quality expectations.
- Must reside in Missouri.
- Active and unrestricted Missouri Pharmacist license
. - Strong knowledge of disease-state management and the 5 Core Elements of MTM Services
. - 1+ year of experience working remotely/independently in a cross-functional environment.
- Ability to navigate MTM platforms and multiple applications simultaneously.
- Strong communication, documentation, organization, and time-management skills.
- Intermediate proficiency with Microsoft Word, Outlook, and Excel.
- PharmD
. - 2+ years of pharmacy experience in retail/community pharmacy and/or PBM; new graduate experiential experience may be considered.
- Previous call center experience.
- Diabetes-related certification.
- Bilingual in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese.
- Additional active Pharmacist licenses in AL, , KS, KY, LA, MD, MN, ND, OK, OR, TN, VT, or WV
