Mental Health Therapist

As a MHT, you will play a key role in the care of clients served within the North Zone. You will be a member of a mobile/virtual interdisciplinary team providing consultation services for palliative and complex care clients. By using a client and family-centered care approach, you will work with local clinicians and other teams to provide specialized assessment, care planning and follow-up for clients in a consultative and limited direct service model.
In this role you will provide coaching and education to other care team members to build capacity in the care of client with palliative and end of life care needs. You will liaise with representatives in Continuing Care, Home and Community Care, Acute Care, community agencies & others for assessment and care planning. You will use information gathered from direct care staff to identify system and client specific barriers and provide leadership in system navigation.
This position supports clients in the North Zone, located out of Thorhild. This role can accommodate working from home; however, you must be willing and able to travel to locations in the North Zone – approximately a 2 hour distance from your home site. Able to work independently in different environments (weather conditions) and must be capable of carrying a medical bag of approx.
25-30lbs.

As the Mental Health Therapist, you will be required to use a variety of treatment modalities (individual, family, and group) to persons experiencing significant mental health difficulties. Working both independently and as a member of a collaborative multidisciplinary team, you will provide evidence informed and best practices in crisis intervention, assessment, consultation, referral, case management and treatment.
