Home-Based Physical Therapist

The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, aligned with professional standards and ethical practices.

• Conducts evaluations and administers treatments such as muscle strengthening, gait training, ADL training, and management of orthotic/prosthetic devices.
• Performs initial and ongoing patient assessments, developing and adjusting individualized treatment plans with clear goals.
• Maintains accurate, timely clinical documentation in compliance with departmental and payer requirements.
• Educates patients and caregivers on activities and exercises to support recovery and independence.
• Communicates regularly with physicians and healthcare team members regarding patient progress and care adjustments.
• Manages appointment scheduling efficiently, including travel logistics, and maintains necessary therapy equipment and supplies.
• Adheres to departmental policies, safety protocols, and quality improvement initiatives.
• Monitors insurance coverages and patient authorizations, reporting concerns to management.
• Performs additional duties as assigned, demonstrating productivity and professionalism.

• High School Diploma or GED;
  Doctor of Physical Therapy (DPT) required, with post-2015 credentials.
• Licensed Physical Therapist and valid Texas Driver's License.
• Auto liability insurance and Basic Life Support (BLS) certification within 90 days.
• Reliable transportation is essential.
