Bilingual Community Health Worker; CHW

Renacer Foundation is hiring Bilingual Community Health Workers (CHWs) to serve older adults throughout Central Florida.

This is a full-time, field-based position
. CHWs manage an assigned caseload of participants and conduct services primarily in participants' homes and community settings.

You will conduct home visits, complete required assessments, identify social needs and barriers, coordinate approved services, provide health education and non-clinical emotional support, and maintain accurate documentation.

Services may include:

• Home visits and participant assessments
• Health education and coaching
• Non-clinical emotional and wellbeing support
• Medical appointment assistance and accompaniment
• Transportation coordination
• Meal and food support coordination
• Basic home cleaning assistance
• Medication routine support and medication organization within CHW scope
• Healthcare and benefits navigation
• Referrals and connection to appropriate services
• Coordination with healthcare providers, social workers, caregivers, and community partners
• Same-day case notes and required program documentation

CHWs do not diagnose medical conditions, prescribe medications, provide clinical treatment, perform clinical procedures, or make medical decisions for participants.

This position is ideal for someone who is compassionate, dependable, organized, comfortable working independently, and able to follow established program protocols.

Because you will spend much of your day in the community, you must be comfortable driving throughout Central Florida, entering participants' homes, managing your assigned workload, and communicating professionally with participants, families, healthcare professionals, and your supervisory team.

• Fluent in English AND Spanish – REQUIRED
• Must have a personal laptop – REQUIRED.
• High School Diploma or GED
• Valid driver's license
• Reliable personal transportation
• Valid automobile insurance
• Ability to travel throughout Central Florida
• Ability to successfully complete a Level II background screening
• Ability to work independently in community-based settings
• Ability to maintain participant confidentiality and comply with HIPAA and organizational policies
• Ability to complete accurate and timely documentation
• Ability to conduct home visits in a variety of environments
• Ability to carry materials weighing up to 20 lbs.

• Community Health Worker certification
• Previous CHW, patient navigation, case management, social services, home-based services, or community health experience
• Familiarity with HIPAA, participant confidentiality, and community-based care

CHW certification is not required for qualified entry-level candidates. Candidates entering through Renacer Foundation's development track must complete required training and commit to obtaining CHW certification within the established timeframe.

Monday through Friday, 9:00 AM–5:30 PM
40 hours per week guaranteed

This is not a remote or office-based position. CHWs travel throughout their assigned Central Florida service area to participants' homes, medical appointments, healthcare facilities, and community locations.

• Starting Pay: $18.00/hour
• Mileage Reimbursement: $0.445 per mile
• Employment Status: W-2
• 40 guaranteed hours per week

A hospital discharge does not always mean that a person's challenges are over.

Some participants return home without transportation, adequate food, household support, a clear understanding of available services, or someone to help them navigate the next steps.

Our Community Health Workers help bridge that gap.
