About the Role
Our client is seeking a seasoned and motivated Senior Social Worker to lead community health initiatives from a remote-first perspective. Based remotely but serving the Port Elizabeth community, you will be responsible for developing and implementing social programs focused on health education, access to care, and well-being within underserved populations. This role requires a proactive approach to identifying community health needs, building partnerships with local health providers, and coordinating outreach efforts.
You will leverage technology to manage projects, engage stakeholders, and ensure the effective delivery of services across the region, contributing to improved health outcomes and community resilience in a flexible, remote environment.
- Lead the design and execution of community-based health and wellness programs.
- Conduct needs assessments to identify social determinants of health within the target population.
- Develop and manage relationships with local clinics, hospitals, and health organizations.
- Coordinate volunteer efforts and provide guidance to community health workers.
- Monitor program impact, collect data, and prepare comprehensive reports for stakeholders.
- Advocate for improved health services and policies within the community.
- Master's degree in Social Work, Public Health, or a related field.
- Registered with SACSSP with a minimum of 5 years of post-qualification experience.
- Proven experience in community health program management and development.
- Strong understanding of public health principles and social determinants of health.
- Excellent project management, communication, and reporting skills.
- Proficiency in remote collaboration tools and digital communication platforms.
