We create environments where people can thrive-where health, safety, dignity, autonomy, social connection, and quality of life are central to everything we do.

We combine caring service with innovative healthcare solutions, including telemedicine, remote monitoring, advanced diagnostics, rehabilitation, behavioral health support, and customized wellness programs. By continuously improving our practices, we help older adults achieve better outcomes and live their best lives.

  • Provide leadership on CMS requirements, Social Services-related F-Tags, resident rights, abuse prevention, PASRR, care planning, behavioral health, and trauma-informed care.
  • Support survey preparation, complaint investigations, Plans of Correction, regulatory recovery efforts, and IDR/IIDR activities when appropriate.
  • Coach and mentor facility Social Services Directors through onboarding, competency validation, education, performance improvement, and career development.
  • Champion resident satisfaction, family engagement, resident and family councils, behavioral health programming, and quality-of-life initiatives.
  • Monitor performance indicators including readmissions, successful community discharges, grievances, satisfaction, care-plan compliance, behavioral health outcomes, and survey trends.
  • Lead Social Services-related QAPI initiatives and use data to identify and sustain improvement opportunities.
  • Partner with Operations, Nursing, Therapy, Admissions, Business Office, Human Resources, and medical providers to create a seamless resident journey.
  • Provide on-site support during leadership vacancies, surveys, transitions, high-risk situations, and operational improvement efforts; serve in an interim facility role when needed.
Minimum Qualifications
