• Provide high-quality physical therapy assessments and treatment plans.
• Document patient progress and outcomes in the EHR with accuracy.
• Adhere to safety, compliance, and professional standards.
• Track clinical metrics and contribute to performance dashboards.
• Collaborate with multidisciplinary team to coordinate patient care.
• Set and monitor patient-specific treatment goals aligned to outcomes.
• Deliver evidence-based therapies and adapt plans as needed.
• Engage in continuous learning and professional development.
• Educate patients and families on home exercise programs.
• Identify opportunities to optimize treatment pipelines and workflows.
• Participate in hybrid work schedule and maintain accountability for outcomes.

• Licensed Physical Therapist with current state licensure and active CPR/First Aid.
• Proven ability to perform comprehensive PT assessments and develop evidence-based treatment plans.
• Accurate documentation of patient progress in EHR, with timeliness and data integrity.
• Deliver evidence-based therapies with flexibility to adapt plans as needed.
• Ability to monitor clinical metrics and contribute to performance dashboards.
• Strong collaboration with multidisciplinary teams to coordinate patient care.
• Set and track patient-specific goals aligned to measurable outcomes.
• Commitment to continuous learning and professional development.
• Educate patients and families on home exercise programs with clear instruction.
• Identify workflow and pipeline optimization opportunities to improve care delivery.
• Participate in a hybrid work schedule and maintain accountability for outcomes.
• Experience with flexible contract roles and ability to manage autonomous performance.
• Comfort with high-visibility work environment and frequent performance feedback.
