Hartford Health Care is seeking a full-time Nurse Practitioner or Physician Assistant to join the REACH program delivering primary care in geriatric patients in their homes and in assisted living facilities around greater Hartford, CT.
You will work with a multidisciplinary team to provide comprehensive care, manage treatment plans, and support families. NP/PA candidates must have CT licensure or eligibility and experience in geriatrics or primary care; two years preferred.
