RADIAS Health is seeking a Case Manager in Minnesota to provide person centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders. You will develop treatment plans, coordinate access to medical, vocational, social, educational, housing and other services, and support clients to live as independently as possible.
The role is 40 hours per week in a hybrid model (community visits and remote work).
