Certified Home Health Aide (CHHA) – Full-Time | Field-Based
Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details- Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
- Occasional evening/weekend overtime available
- Field-based position with travel required (mileage reimbursed)
- Active CHHA certification required
Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands-on care and support, including:
- Assisting with mobility, transfers, and ambulation
- Providing personal care (bathing, grooming, dressing, feeding)
Offering companionship and emotional support - Performing light housekeeping tasks
- Documenting care accurately and in a timely manner
- Supporting a culture of compassion, quality, and respect
You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.
What We’re Looking For- Active Certified Home Health Aide (CHHA) certification
- Valid driver’s license, reliable vehicle, and current auto insurance
- Compassionate, reliable, and self-directed
- Strong communication and time-management skills
- Ability to work independently and follow care plans
- Commitment to serving patients with chronic, serious, or terminal illnesses
