Center Well Home Health, part of Humana, is seeking a full-time Home Health Physical Therapist to deliver personalized in-home care, improve functional outcomes, and help patients regain strength, mobility and independence in the comfort of their own homes.
You will plan and administer PT treatment, assess patient strengths, develop rehab programs, supervise PTAs and home health aides, document progress and communicate with physicians, the clinical team, patients and families.
