Elara Caring is seeking a Physical Therapist to serve patients throughout the Danbury, Connecticut territory. In this field-based role, you will help patients regain strength, mobility, and independence in the place they feel most comfortable: home.

* Evaluate patients’ mobility, strength, safety, and functional needs
* Develop and implement individualized treatment plans
* Help patients improve mobility and safely perform daily activities
* Educate patients and caregivers on exercises, equipment, and safety techniques
* Monitor patient progress and adjust treatment plans as needed
* Communicate changes in condition to physicians and members of the care team
* Complete accurate and timely clinical documentation
* Collaborate with nurses, physicians, therapy professionals, and other team members
* Support patients in reaching their highest possible level of independence## ##

What You’ll Bring
* Graduate of an American Physical Therapy Association-approved physical therapy program
* Active and unrestricted Connecticut Physical Therapist license
* At least one year of experience in a healthcare setting
* Strong communication, organization, and patient-care skills
* Ability to work independently in a home-based environment
* Willingness to travel throughout the assigned Danbury territory
* Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
* Must be able and willing to travel 50%
* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
