Our client is seeking a dedicated and experienced Remote Case Manager to join their health services team. In this role, you will remotely support individuals in navigating their healthcare needs, coordinating services, and ensuring access to appropriate medical and social resources. You will act as a central point of contact, working closely with clients, healthcare providers, and community agencies to develop comprehensive care plans.
