Lins Care is seeking an experienced Occupational Therapist to join the Adult Social Care Hospital Discharge Team in London. You will conduct specialist OT assessments, identify functional needs and risks, and recommend equipment and adaptations to enable safe discharge.
Working with multidisciplinary colleagues, you will support timely discharge, reduce delays and promote independence for adults with health and social care needs. HCPC registration is required.
