Job DescriptionRD Nutrition Consultants LLC is seeking a Registered Dietitian Nutritionist to provide nutrition care at a skilled nursing facility in San Bernardino, CA. This part-time opportunity offers a blend of onsite patient care and remote work, with ongoing support from our dedicated team.
Facility Type: Skilled Nursing Facility
Schedule: 8-12 hours per week
Remote Flexibility: Up to 50% of weekly hours may be completed remotely.
Compensation: $40.00-$45.00 per hour, depending on experience.
Key Responsibilities- Complete comprehensive nutrition assessments and follow-ups for residents identified at nutritional risk.
- Develop, implement, and monitor individualized nutrition care plans.
- Monitor weight trends, oral intake, laboratory values, skin integrity, and other indicators of nutritional status.
- Recommend appropriate nutrition interventions, including therapeutic diets, oral nutrition supplements, and enteral nutrition support as indicated.
- Collaborate with nursing, dietary staff, and the interdisciplinary care team to support resident health and quality of life.
- Provide nutrition education and counseling to residents, families, and caregivers.
- Maintain accurate and timely documentation in the facility’s medical record system.
- Participate in care conferences, quality improvement activities, and nutrition-related meetings as needed.
- Follow facility policies and applicable standards for nutrition care in a skilled nursing setting.
- Perform additional duties within the dietitian’s scope of practice as needed.
- Current registration with the Commission on Dietetic Registration (RD/RDN).
- Bachelor’s or master’s degree in Dietetics, Nutrition, or a related field from an accredited program.
- Previous experience in skilled nursing or long-term care preferred.
- Strong clinical assessment, communication, and organizational skills.
- Familiarity with electronic medical records and interdisciplinary care planning.
- Ability to work independently and manage a part-time hybrid schedule.
- Availability to complete one onsite visit each week in San Bernardino, CA.
