Seamlessassist is hiring a full-time, remote Clinical Pharmacist to lead GDMT optimization for high-risk cardiac patients. You will partner with RN Care Managers, Nurse Navigators and cardiology providers to ensure proper therapy initiation and titration, improving adherence and outcomes.
This is a clinical, non-dispensing role within a value-based care model. A W-2 benefits package applies, with business-hours overlap in Central Time, and full remote flexibility.
