Home-Based Physical Therapist

The OP Home-Based Physical Therapist delivers comprehensive physical therapy services, including assessments, treatment planning, and interventions aligned with professional standards and ethical practices.

• Conduct evaluations and administer treatments based on physician referrals for patients across all ages.
• Provide therapy such as muscle strengthening, gait training, ADL training, and device management.
• Perform initial and periodic assessments, document progress, and develop tailored treatment plans with measurable goals.
• Ensure accurate, timely clinical documentation compliant with departmental and payer guidelines.
• Instruct patients and caregivers on activities and treatment regimens.
• Maintain communication with the healthcare team, including physicians, regarding patient progress and care adjustments.
• Manage appointment scheduling, transportation, and portable therapy equipment.
• Adhere to department policies, safety, and quality improvement programs while managing productivity.
• Monitor insurance coverage and patient authorizations, escalating concerns appropriately.
• Perform additional duties as assigned to support departmental objectives.

• Education:
  
  High School Diploma or GED required;
  Doctor of Physical Therapy (DPT) after 2015.
• Licensure:
  Valid Physical Therapist license, Texas driver’s license, auto liability insurance, and BLS certification (within 90 days).
