Sentara Health Plans is hiring a Care Coordinator for LTSS Waiver Members/Medicaid - Remote in Northern Virginia/NOVA (Culpeper, Riva, Rixeyville, Bealeton, Remington VA) and the surrounding areas!

Status:
Full-time, permanent position (40 hours)

Standard working hours: 8 am to 5 pm EST, M-F

The position requires in person/face-to-face assessments with remote/home office for the LTSS Waiver Members/Medicaid in Northern Virginia/NOVA ( Culpeper, Riva, Rixeyville, Bealeton, Remington VA ), and the surrounding areas! May travel up to 1 hour max.

Candidate must reside in Northern Virginia/NOVA ( Culpeper, Riva, Rixeyville, Bealeton, Remington, VA )and the surrounding areas.

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate in, and monitor multidisciplinary collaboration of services to patients where appropriate. Assists adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including the time of terminal illnesses.

• 1 year of Long Term Care required experience REQUIRED
• Case Management experience preferred
• Long-Term Services and Supports (LTSS) waiver or Medicaid or Health Plan experience preferred
• Behavioral Health experience is secondary.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits.

The base pay range for full-time employment is $24.91 - $40.07 hourly.

Additional compensation may be available for this role, such as shift differentials, on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities.

Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
