Responsibilities:- Provide one-on-one care to patients in their homes, focusing on their physical, emotional, and social needs
- Assist with daily living activities, such as bathing, dressing, and grooming
- Support patients with medication management, meal preparation, and light housekeeping tasks
- Encourage independence and engage patients in activities that promote physical and mental well-being
- Collaborate with our interdisciplinary team to develop personalized care plans and ensure seamless transitions
