Reema Health, Inc. seeks a Community Health Navigator (Spanish Speaking) to work remotely or in the field in the Albuquerque region.
You will build relationships with members, guide them toward greater engagement in their healthcare, and enroll them in Reema's care model, addressing social barriers with empathy. You’ll manage a caseload of 80–120 members, coordinate referrals and appointment scheduling, and document all work in the Reema App.
