Position Summary: The Family Support Services Supervisor is responsible for coordinating all aspects of patient social services(social work, chaplaincy, & bereavement) as well as the area of volunteer coordination and community outreach.

This position is responsible for establishing, implementing, and evaluating goals and objectives tomeet and promote the standards of quality and contribute to the PVHPC mission and philosophy.

Essential Job Functions

• Superviseandcoordinatethe Family Support Services Departmentunderdirectoversight of the Executive Director. The Family Support Servicesteamconsistsofthe Volunteer Coordinator, Bereavement Coordinator, Chaplains,Social Workers,and Integrative Therapists.
• Develop and implementtheobjectives, policies, procedures,and job descriptions forthe
  
  Family Supports Services Department.
• Ensure thatall
  
  Family Support Services are delivered consistently with theplan of careandin accordance with standards of practice.
• Ensure compliance with all state, federal,and CHAP regulatory requirements.
• Coordinate the implementation of improved work methods and procedures to ensure services are delivered in accordancewithpolicy.
• Ensure seamless care service delivery by providing oversight ofthecoordination of care with all clinical service providers.
• Assistinrecruiting,selection, orientation, evaluation,and directlymanagemembers of the Family Support Services Team.
• Coordinate and supervise the assessment, planning, implementation,and evaluation ofthe clinical care by members of the Family Support Team.
• Assure proper documentation in clinical recordscomplieswithstate, federal,and CHAP standards.
• In conjunction withthe

  Director,provide leadership in strategic planning, including identifying opportunities foradditionalor improved services to address unmet customer needs.
• Maintainacomprehensive working knowledge of the field of social work andshareinformation with appropriate organizational personnel.

  Family Support Services are delivered according to contractual provisions.
