You will deliver nutrition education via telephonic and video visits, set SMART goals, monitor progress, and guide lifestyle changes to support heart health and blood pressure management. Up to 40 hours per week, with shifts 8:00 a.m.
to 5:00 p.m. Monday–Friday. Responsibilities include comprehensive nutrition assessments, care planning, and collaboration with the interdisciplinary healthcare
