Occupational Therapist (OT) Home Health West Portland, OR and Surrounding Areas. This position involves providing skilled occupational therapy services to patients within a home health setting. The role requires a valid Oregon license, with the potential for a start date as soon as possible. The scheduled shifts are during the daytime, with a standard 8-hour workday, and the position offers a consistent weekly schedule.

  BCLS certification is required.
• Education & Experience Requirements:
  Must possess an Oregon license and have at least 1 year of skilled home health experience; first-time travelers are accepted.
• Schedule:
  
  8:00 AM to 4:00 PM shifts, 40 hours per week.
• Additional Qualifications:
  
  Proficiency in Homecare Homebase (HCHB), experience with Oasis documentation (homecare start of care), and valid driver's license with personal transportation are required.
• Responsibilities:
  Offer occupational therapy services tailored to adult and pediatric patients, following established clinical protocols, documentation standards, and patient care plans. Occasional use of home-based electronic charting tools is mandatory.
• Other Requirements:
  Candidates must have their own transportation, be able to visit up to 6 patients per day, and adhere to safety and clinical standards. RTO (Requested Time Off) must be submitted during application, and an active driver’s license is required.
• Additional Notes:
  No float required; limited maximum radius of roughly 60 miles daily; occasional weekend availability not required.
