Key Responsibilities
- Assess functional abilities and needs to determine therapy goals.
- Develop and implement individualized, person-centered care plans.
- Provide restorative occupational therapy to improve daily living skills.
- Recommend adaptive equipment and strategies to support independence.
- Evaluate and modify home environments to reduce barriers and improve safety.
- Collaborate with nurses, physicians, social workers, and caregivers.
- Document patient status, progress, and outcomes.
- Educate participants and families on care plans and home recommendations.
- OT licensure and strong clinical judgment in functional assessment.
- Experience with home-based or community-based therapy and environmental modification.
- Ability to create compassionate, dignity-focused interventions.
- Effective communication and teamwork across an interdisciplinary team.
- Strong documentation skills and commitment to participant-centered care.
