Remote​/Hybrid Clinical Prescribing Pharmacist

This exciting new role provides the opportunity to work with the General Practice and alongside a team of pharmacists and pharmacy technicians to support the delivery of pharmacy services in general practice.

You will be part of a dynamic, forward-thinking, pharmacy team delivering medicines management across a PCN with the support of a named pharmacist.

The team comprises a clinical pharmacist, pharmacy technicians, a Lead Pharmacist and GP clinical colleagues, providing appropriate clinical supervision, support and multidisciplinary working.

You will be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews, responding to medication queries, signing prescriptions, and to proactively manage people with polypharmacy and multi-morbidities.

You will have the opportunity to attend meetings at practice, PCN, and ICB level, deliver training to practice and pharmacy team colleagues, mentor students
