Humana is seeking a Care Manager, Telephonic Behavioral Health, to assess and support members’ wellness remotely. You will evaluate needs, guide families to resources, and coordinate with care teams to ensure smooth transitions of care, including TOC initiatives after hospital discharge.
Remote role with occasional travel (5-10%) and a requirement to reside in South Carolina. The schedule is Monday–Friday, 8:00 AM–5:00 PM ET, with comprehensive benefits and a performance-based bonus eligible
