Position: Case Manager (part-time)

Push for Prosperity is seeking an experienced Case Manager to support participants enrolled in a grant‑funded workforce development program. This independent contractor role is ideal for professionals passionate about career pathways, participant advocacy, and measurable employment outcomes.

Position Summary

The Case Manager is responsible for coordinating, managing, and documenting services provided to program participants from initial engagement through job placement. This role ensures participants receive comprehensive, high‑quality support aligned with program goals and grant requirements.

The Case Manager conducts assessments, develops individualized development plans (IDPs), connects participants to appropriate services and resources, and tracks all activities and outcomes in the Workforce One (WF1) system on a weekly basis. This position manages a high-volume caseload and plays a critical role in participant retention, engagement, and successful workforce outcomes.

Key Responsibilities Participant Assessment
  • Conduct comprehensive intake assessments to evaluate participants’ needs, strengths, barriers, and career readiness.
  • Perform ongoing evaluations to address immediate and long‑term needs throughout program participation.
Individual Development Planning
  • Develop and maintain individualized development plans (IDPs) aligned with participant goals and career pathways.
  • Establish measurable goals, milestones, and outcomes.
  • Update IDPs regularly based on progress, challenges, and changing needs.
Service Coordination & Referrals
  • Connect participants to internal supports, community resources, and partner services.
  • Coordinate with mentors, training providers, and partner agencies to support participant success.
Monitoring, Engagement & Retention
  • Maintain regular contact with participants to ensure continued engagement and enrollment.
  • Monitor progress and intervene as needed to address attendance, participation, or employment barriers.
  • Conduct post‑cohort IDP check‑ins to support job search and placement activities.
Documentation & Data Management
  • Maintain accurate, timely, and complete documentation of all participant interactions, assessments, services, and outcomes.
  • Enter case notes, activities, and required documentation into the WF1 system on a weekly basis.
  • Ensure all program data meets grant, legal, and organizational standards.
Compliance & Confidentiality
  • Ensure compliance with all applicable laws, regulations, grant requirements, and organizational policies.
  • Maintain participant confidentiality and ensure data security for all records and documents.
  • Stay current on best practices and regulatory updates related to case management and workforce development.
Key Performance Indicators (KPIs)

The Case Manager is accountable to assess the following deliverables:

  • Recruitment & Enrollment (20%):
    Recruit and enroll a minimum of 12 candidates per cohort term; provide weekly recruitment updates to the Workforce Lead.
  • Onboarding & Documentation (30%):
    Ensure 100% of applicants are fully onboarded with all required documentation uploaded into WF1, completed no later than four (4) days prior to cohort start.
  • Grant Partner Coordination (15%):
    Meet with Grant Mentor Partner at least twice per cohort to review and audit candidates in WF1; provide updates to the Workforce Lead.
  • Ongoing Recruitment Activities (15%):
    Conduct online recruitment (Handshake or similar platforms) or grassroots outreach at least once per week or as events present themselves; generate 1–2 qualified candidates per program.
  • Participant Engagement & Case Management (20%):
    Maintain enrollment and engagement of all cohort participants in WF1; complete weekly case notes, updates, and service documentation; conduct post‑cohort IDP job search check‑ins within one week of cohort completion.
Qualifications
  • Bachelor’s degree in social work, psychology, human services, or related field preferred; equivalent experience considered.
  • Minimum three (3) years of case management, workforce development, or social services experience.
  • Strong interpersonal, communication, and relationship‑building skills.
  • Ability to manage a high-volume caseload independently.
