A reputable and mission-driven home care agency is seeking a Director of Coordination to lead a dynamic team of professionals supporting hundreds of clients across NY.
Home Office Flexibility:
The position is based out of either our Brooklyn or Spring Valley, NY office. The selected candidate may choose their preferred home office and will travel to the other location approximately once per week.

About the Role
You'll lead a high-performing team responsible for caregiver scheduling, service coverage, client relations, payroll accuracy, and daily operations. This hands-on leadership role is ideal for someone who can inspire others, think operationally, and maintain a calm, solutions-oriented mindset in a fast-paced setting.
Key Responsibilities
- Supervise and support coordinators managing caregiver schedules and daily service delivery
- Oversee full shift coverage, including last-minute or emergency staffing
- Drive efficient, accurate caregiver-patient matching
- Ensure compliance with agency standards, DOH regulations, and documentation timelines
- Monitor service delivery and implement corrective actions when needed
- Cultivate a culture of accountability, teamwork, and care
Requirements
- At least 3 years of leadership experience in home care coordination or HR, in a LHCSA
- Experience managing high-volume, multilingual teams (on-site and remote)
- Skilled in communication, coaching, scheduling, and compliance oversight
- Proficient in Microsoft Office and scheduling tools (e.g., HHAeXchange, Smartsheet)
Preferred:
- Multilingual (Spanish or Yiddish)
- Knowledge of Medicaid waiver programs
- Experience in quality assurance, internal audits, or process improvement
