PositionFull Time Registered Nurse Case Manager and Part Time opportunities available

Territory: Citrus Heights, Fair Oaks and surrounding areas.
What you’ll do as a Registered Nurse (RN) Home Health Case Manager- Utilize clinical skills to create, implement, and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
- Oversee patient and family/caregiver teaching with wound care, blood draws, tube feedings, Tracheostomy care, foley catheters, CHF management, and medication education in the patient’s home.
- Perform a full assessment of the patient and family on initial visit.
- Use assessment data to develop an advanced plan of care including rehabilitation.
- Work with patients and families one-on-one to achieve improvement and/or stability.
- Case manage the patient’s care episode with an interdisciplinary clinical team.
- Prepare clinical notes, orders, and charting within a compliant timeframe.
- Collaborate with physicians and agency staff to deliver the best care possible.
- Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols.

- Competitive compensation with an achievable incentive bonus program.
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- High level of clinical knowledge in home health care (highly preferred)
- Experience with high acuity patients (a plus) and interdisciplinary teamwork
- Case Management fundamentals (highly desired) and IV medication infusion experience (highly preferred)
- Proficiencies in initiating intravenous (IV) therapy a plus
- Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills

Salary: $130,000 - $160,000 a year. Wage range for Registered Nurse, Level 1: $130,000 – $160,000 plus a $10k annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21
