As part of Strive’s interdisciplinary clinical care team, the Nurse Practitioner will utilize advanced education, skills, and judgment to deliver advanced nursing care for CKD and ESKD patients. The NP will be an advocate, navigator, trusted clinician, and safety net for the patient while delivering exceptional clinical care and a world‑class patient experience. This position reports to the Lead, Nurse Practitioner.
TheDay To Day
- Accountable for patient outcomes and KPIs for a panel of CKD and ESKD patients and collaborates with external providers to ensure goal alignment.
- Obtains patient history, performs physical exams, orders and interprets diagnostic tests, and formulates short‑term and longitudinal care plans.
- Presents patient cases and provides clinical support during rounds and interdisciplinary meetings.
- Ensures assessment and plan incorporate best practices for chronic kidney care, shared decision‑making, and end‑of‑life/advanced care planning discussions.
- Performs patient visits in multiple settings including home, clinics, partner MD spaces, and via telehealth/virtual visits.
- Liaises with dialysis facilities, PCPs, and nephrologists to streamline transitions in care.
- Makes sure to maintain board certification and state‑specific continuing education requirements.
- Adheres to Strive’s documentation policy and delivers primary and urgent care for adult and geriatric patients with CKD history in designated markets.
- Provides in‑person patient care that may require standing, sitting, walking, pushing, pulling, and lifting.
- Master’s degree in Nursing or similar field.
- 2+ years of experience as a Family or Primary Care Nurse Practitioner or in internal medicine, cardiology, nephrology, or case management.
- Current state‑specific licensure required.
- National Board Certification.
- DEA license required or eligible to obtain within 90 days of hire.
- BLS or CPR certification required.
- Valid driver’s license and ability to travel across assigned region to meet patient needs.
- Basic computer and Microsoft Office skills.
- 2+ years of experience with Electronic Health Records (EHR).
- Wound care certification preferred.
- Experience using audio‑visual technology platforms.
- Proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation for complex chronic patients.
