Position Details
- Hybrid‑Remote:
Work from home (telehealth) and travel within an assigned geography for in‑person visits (dialysis clinics and nephrology practices). - Monday‑Friday schedule with flexibility to accommodate patient and nephrology partner’s availability.
- Full‑time position.
- Build empathetic relationships rapidly with patients and families.
- Coordinate patient care: care transitions, management of complex/at‑risk patients, ongoing needs, and establishing a treatment plan in partnership with the care team.
- Identify and address social determinants of health including medical, emotional, educational, community, and financial concerns.
- Coordinate streamlined and effective transition of care when needed.
- Assess patients to identify unreported medical conditions or changes that may lead to adverse outcomes.
Requirements
- Passion for delivering care to patients with complex, chronic illnesses.
- Flexibility and willingness to pivot quickly on priorities.
- Current RN License required; BSN preferred.
- Minimum 2 years RN experience with healthcare performance coaching.
- Current CPR or BLS certification.
- Current driver’s license and willingness to travel within an assigned territory.
- Intermediate computer skills; proficiency in MS Word, Excel & Outlook.
- Home office with internet connectivity: minimum 1 MB upload & 1 MB download.
Preferred Qualifications
- 2+ years of Case Management or Chronic Care Management experience.
- 1+ year of utilization management experience.
- Spanish‑speaking preferred in some markets.
