** Monterey County, CA region.
**** As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You’ll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients.
** You’ll work autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, consistent, cost-effective care.

Hybrid-Remote. Work from home and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.**
* ** Complete Comprehensive Health Assessments**
* ** Manage complex patients, focused on reducing hospitalizations and improved health outcomes
*** Monday
- Friday schedule with the ability to accommodate patient and nephrology partner’s availability.
** Must be flexible.

** Requirements
* ** Passion for delivering care to patients with complex, chronic illnesses
* ** Minimum of two (2) years' experience
** as a Nurse Practitioner (NP)
* Current unrestricted NP license to practice in the state
* Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
* Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
* Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
* ** Home Office, High Speed Internet and Reliable Transportation Required.
**** Preferred Qualifications
*** Preferred experience working in
** Value Based Care or Population Health including documenting and coding from a MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
*** 5+ years’ experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
* Spanish-speaking preferred in some markets
* Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
* Manage pop. Health (hospitalization)
