Pair Team is building a team of passionate individuals to change primary care operations for those who need it most. We are looking for a highly motivated full-time Bilingual Family Nurse Practitioner who can think creatively and empathetically to help our team change how people access healthcare. This role supports Federally Qualified Health Centers in California participating in CalAIM's Enhanced Care Management Medi-Cal benefit program, providing long-term, whole-person care coordination including behavioral health and social needs supports.
The Bilingual Family Nurse Practitioner will engage and care for individuals living with Serious Mental Illness/Substance Use Disorder, homelessness, and/or high medical needs. The Lead Care Manager works closely with partner clinics, community organizations, and the Pair Team clinical team to build trusted relationships and deliver comprehensive care.
Responsibilities- Provide best-in-class virtual preventive care for underserved patients using internal care delivery technology and clinical judgment
- Develop and refine clinical programs to support patients holistically and address barriers to care
- Collaborate with product and care teams; provide feedback to improve the platform
- Work with a diverse, inclusive team
- Remote work with equipment provided
- 2+ years of clinical experience serving patients with complex social and healthcare needs
- 1+ year of primary care experience
- Experience or desire to work with a multidisciplinary team
- Technology-savvy; comfortable with telemedicine and multiple tech platforms
- Board certification or eligibility in Family Medicine
- Comfortable working with patients across the lifespan (pediatrics to geriatrics)
- Experience in Medicaid/FQHC clinics
- Bilingual in Spanish and English
- NP license in California
- Active DEA license preferred
- Startup experience is a plus
- Competitive salary: 125k–135k base
