Summary:
Under the direction of the Hospice & Palliative Care Manager and within established policies, responsible for performing skilled nursing care to patients/families in accordance with the State Nurse Practice Act and accepted professional standards. The RN delivers care as delegated by the RN Case Manager (RN/CM) and/or Hospice & Palliative Care RN Coordinator (HPC-RNC). This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson VNA & Hospice.
Must be able to interact effectively with individuals at various levels of intellectual and physical ability (adolescent, adult, pediatric).
Responsibilities- Provide professional nursing care under the direction of a RN/CM, HPC-RNC or Hospice & Palliative Care Manager.
- Provide skilled nursing visits to patients and families, including informational or evaluation visits, admission visits, re-visits, or discharge visits.
- Participate in the development of the care plan with Interdisciplinary Group (IDG) members.
- Follow the plan of care and carry out interventions established by the RN/CM.
- Recognize and report changes in patient condition to the RN/CM or CTM as directed.
- Communicate information in a timely manner, consistent with policy and urgency of the situation.
- Provide care within the scope of clinical practice and competency, including assessment, medication monitoring and administration, patient/family teaching, and emergency intervention.
- Perform skilled nursing services in homes, SNFs, hospitals, and other community settings.
- Provide on-call support for patients based on needs and policy; rotate weekends, evenings and holidays as required.
- Supervise Home/Hospice Care Aides in accordance with agency policy.
- Maintain competency with nursing skills and adhere to legal scope of practice and agency policies.
- Maintain accurate clinical records and document interventions, responses, and evaluations.
- Promote safety and infection control; maintain environmental standards.
- Participate in mandatory safety education and quality improvement activities.
- Adhere to state and federal laws and agency policies; meet visit productivity standards.
- Attend meetings, maintain confidentiality, and engage in ongoing professional development.
- Keep informed of changes in hospice/home care regulations and reimbursement requirements.
- Maintain required certifications for the per diem RN position as applicable.
- Education:
Nursing Diploma/Certificate (required) or Bachelor’s Degree in Nursing preferred. - Licenses and Credentials: RN license in Massachusetts; valid Class D Driver's License (MA) or equivalent; good driving record.
- Experience:
Minimum of one year in a home care/hospice setting or equivalent; hospice experience preferred. - Knowledge, Skills, and Abilities:
Graduate of an approved nursing program; knowledge of home health/hospice regulatory and reimbursement requirements; strong verbal and written communication; organizational and time-management skills; reliable transportation. - Physical Requirements:
Ability to stand, walk, sit, lift, push/pull, reach, manipulate, and perform tasks as listed in the job posting; relevant vision, hearing, and speech capabilities; ability to travel as needed.
